It is more accurate to view the division of life-cycle into phases as a management technique, rather than as a model of how the system development actually proceeds. There are many problems associated with Requirements Engineering, including defining system scope, problems in fostering understanding different stakes involved, and problems dealing with the volatile nature of requirements.
